How Netflix’s High Flying Bird Upends the Conventions of the Sports Movie

There’s no game-winning miracle dunk in High Flying Bird, a new basketball film from Steven Soderbergh that arrived on Netflix on Friday. There’s no training montage, rousing halftime speech or pint-sized surprise hero, either.

Instead, the film is driven by backroom machinations, Sorkin-esque walk-and-talks and tense face-offs over cups of tea.

But while the film mostly lacks basketball, it is more true to the state of modern professional basketball than most other films about the sport—and it strikingly captures the current power struggle of black athletes as they battle with owners for player autonomy, free speech and billions of dollars in revenue.

The film follows the fictional agent Ray Burke (André Holland) in the midst of the contentious 2011 NBA lockout. He works to outmaneuver a cutthroat team owner (Kyle MacLachlan) in lockout negotiations, expand the mindset of a downtrodden, debt-ridden rookie (Melvin Gregg) and team up with a steely player’s union executive (Sonja Sohn) to alter the economic structure of the league.

The stakes may initially appear lower than other Soderbergh films—like drug trade in Traffic or corporate corruption in Erin Brockovich. But the heart of the movie’s conflict lies in the control and commodification of black bodies. One character compares the NBA draft to a slave auction; another recounts the NBA’s white-only origins, describing the league’s integration in 1950 as a “game on top of a game”: a system used by wealthy white owners used to control players’ movements, image rights and earnings.

The fierce and dense screenplay was written by Tarell Alvin McCraney, who won an Oscar in 2016 for co-writing Moonlight. Like that film, High Flying Bird champions characters who search for radical ways to survive and transcend unjust systems. McCraney explained a driving factor behind both films in an interview with GQ this month: “On one hand, the American dream is being carroted in front of us, but on the other, the stick of oppression is beating us.”

McCraney, Soderbergh and Holland (who co-produced the film) situate the film within a lineage of black protest. It takes its name from the Richie Havens version of a folk song that poignantly calls for freedom. And Ray treats the sociologist Harry Edwards’ 1969 book The Revolt of the Black Athlete as a sacred text. In that book, Edwards outlines the systematic discrimination faced by black athletes and recounts his efforts to create a black boycott of the 1968 Olympics, which led to a Black Power salute in Mexico City. “They tell the world that the Games are free of discrimination, a wonderful example of fair play to everyone,” he writes. “Meanwhile, neglect kills off your people faster than you can sprint.”

Edwards himself appears in High Flying Bird, forging a direct link between a time when black superstars like Bill Russell had to sleep in separate hotels and a new era of protest. Edwards now serves as Colin Kaepernick’s advisor and works with many sports teams; he remains vocal about what he terms the “social, physiological and cultural scaffolding that allows individual bias and prejudice to find affirmation in discriminatory actions.”

In the same way that Edwards worked to debunk the rosy vision of sports presented by the Olympics, McCraney and Soderbergh use High Flying Bird to rebel against the utopian construct of sports movies. Films like White Men Can’t Jump, Glory Road and The Blind Side propagate the idea that sports can drive equality; that class and race tensions vanish while on the hardwood or gridiron through a shared determination and perseverance.

High Flying Bird, in contrast, is far more cynical. “The league is a business,” Ray reprimands Erick. “Business. We are in business.” While Michael Jordan won his freedom through a buzzer beater in Space Jam, High Flying Bird quashes the notion that on-court victory even matters. The film’s NBA isn’t a conduit for greatness but rather a cold, unfeeling corporation in which MacLachlan’s snot-rocketing executive profits off of black men scraping against each other in a zero-sum game. High Flying Bird could be called an anti-Sports Movie: its goal is not to uplift, but rather to provoke, mobilize and envision a future in which the players themselves own the league. And in contrast to the sweeping cinematography of other sports films, High Flying Bird was shot on an iPhone.

In real life, the 2011 lockout ended in relative defeat for the players’ union, as NBA owners forced players to accept a reduction in their share of revenue. But since then, players have taken steps to increase both their income and agency, drastically changing the landscape of the league.

LeBron James, in particular, has been revolutionary in how he wields power over his own career. He has encouraged other players to follow his lead in claiming autonomy, and he condemned a double standard that shackles devoted superstars to teams while allowing owners to trade them when it suits their business strategy. (On Wednesday, he took to Instagram to criticize the way in which Harrison Barnes was unceremoniously shipped off for a trade during a game.) This season has seen several stars—including Jimmy Butler and Anthony Davis—use their leverage to forge their own career paths rather than stay beholden to team owners.

These efforts have been aided by the rise of social media and other online outlets that allow players to control their own public image and speak out on political and social causes. James famously tangled with Donald Trump on Twitter, while Kevin Love opened a dialogue about mental health issues on The Players’ Tribune, a media platform founded by Derek Jeter.

Meanwhile, a massive $ 24-billion TV deal, combined with a favorable 2017 bargaining agreement negotiated by Michele Roberts, the leader of the N.B.A.’s players union, landed huge payday opportunities for young stars, 45 percent raises for players on minimum contracts and higher minimum salaries for veterans.

In High Flying Bird, Ray aims even higher, dreaming of a radical player-owned league in which games are streamed straight to YouTube or Netflix. Such a drastic shift seems unlikely any time soon. Until then, activists, filmmakers, and the players will continue to work to challenge power structures and shake the perception that athletes are not looked at as “super animals,” as Edwards wrote in 1969, but treated with full humanity.

How to Keep Your Spirits High When Life Stinks

There was an interesting reader threadjack recently about how to keep morale high when your confidence is wrecked and everything feels like it’s going wrong, either at work, in life, or everything. I’m going through one of these times myself at the moment (it’s been a trying week, mostly for complicated kid-related reasons I won’t get into) but… yeah. Sometimes things stink. So, I’d love to hear your suggestions — and here are ideas that have been suggested in the past…

  • Enforce boundaries. If things are going wrong at work, make sure that your home is your haven — we’ve talked in the past about how to build work-life boundaries as well as to how to keep your work life separate from your personal life. Some of our tips on how to focus on work may also be of help
  • Give your brain a break. If things are going wrong and you’re dwelling on them, we’ve talked about how to stop overthinking. We’ve also discussed how to help your brain switch gears by focusing on your senses (also helpful to turn a bad day around!) — go shopping and touch the different fabrics as you pass, or consider baking bread to knead/pound/have sensory fun playing with something gooey.
  • Focus on what you need to. Don’t make things worse by letting things slide. Whether it’s life or work that are making things horrible, we all still have our daily obligations. Picking a different workspace to work in (like a conference room or even just the visitor’s side of your desk) can help you focus on work when other things threaten to distract you.
  • Get your cardio in. I’m desperate to fit a run in at some point today because the weather was too poor to do it last week and I feel like it’s exacerbating everything. If you’re under the gun with a work project and you have no time, try to fit in cardio to get your heart rate up — take a 5-minute break to climb stairs at your office, do pushups on the floor, or if you have the space (and a private office) try a few kettlebell swings. As we’ve noted before, dancing or singing to silly music can also be a great pick-me-up and a great break to build into your day. (Perhaps I need to change my alarm song — right now it’s Gossip’s 2012. I’m off to review our last discussion on everyone’s favorite girl power songs…)
  • Laugh. We haven’t had a thread on this in far too long: What are your favorite hilarious things to perk you up, ladies? (I am not really an animal person, but for those of you who are, please share your favorite memes/kitties/etc.). I died laughing at the Peloton Twitter thread that went viral, but so much comedy (late night and SNL) is tinged with politics right now that it often makes me more stressed out when I’m in those moods. Some of the classics (for example, “Baby Trashes Bar” or  “Mean Kitty“) are still some of my favorite light-hearted YouTube things.
  • Make a list. There are two ways to do this: One option is to make a list of your accomplishments, the things that are going right, and/or the things you’re grateful for. The OTHER option is counterintuitive, but I’ve found it’s helped me sometimes — make a FUBAR list, noting every single thing that is dragging you down. Doing a brain dump really helps, and it’s always kind of amusing after the cloud has lifted to go back and see EVERYTHING on your list because you realize that most of it doesn’t matter… then the next time the mood hits you can sort of remember how you laughed at everything after the fact.
  • Mini-meditation. Meditation has all sorts of benefits, but it can take time and discipline to feel like you’ve getting benefit from a meditation practice. This 60-second meditation is great, and we talked about some other helpful meditation apps a few months ago (like the Apple Watch Breathe app).
  • Enforce the 24-hour rule. This is a good guideline for weight loss, but it also works if you’re having a lousy time with a coworker, a boss, a life situation… try to envision what a “good” next 24 hours look like for you, and then commit to making it happen. In weight loss parlance that may mean pre-planning your meals; for work it may mean pre-planning your day so you build in cardio, a dance party, a YouTube break, or other mood-boosters.

Felicity Jones Talks Flying High at Dior’s Circus

ROLL UP, ROLL UP: One of the guests at the circus tent setting for the Christian Dior show Monday afternoon felt right at home. “Rogue One” actress Felicity Jones had just a week ago wrapped filming “The Aeronauts,” in which she plays a circus performer and hot air balloon pilot.
“It’s a real magical adventure story, so this is totally fitting,” she said. Even the acrobatics seen during the show were nothing to scare the rising performer — the role saw her shooting in a hot air balloon.
“It’s pretty amazing, actually, because you go up so gently it’s not quite as scary as you think it’s going to be because it’s a slow incline, but then suddenly you’re 2,000 feet up looking out of the balloon and thinking, ‘Wow, it is actually quite high up here.’”
Maria Grazia Chiuri had called on acrobatic troupe Mimbre to perform as part of the circus-inspired show, framing the models as they walked the runway. “She wanted Mimbre to come with a team of all-female acrobats,” Lina Johansson, artistic director and cofounder of the London-based company, who began work choreographing the project with 18 performers in November, explained backstage before the show.

Fortune 500 Company Corporate Board Diversity at All-Time High

A new report from the Alliance for Board Diversity (ABD) and Deloitte, reveals that women and people of color represent 34% of all corporate board seats in Fortune 500 companies—placing board diversity at an all-time high.

Here are some key findings from the study:

-Black woman gained 32 board seats in 2018, an increase of 26.2% from 2016.

-Black men gained 26 board seats in 2018, an increase of 8.5% from 2016.

-Black and Asian women achieved the largest increase in board seats; black women at a 44.8% increase, and Asian women at a 30.8% increase.

-Companies are increasingly re-appointing minority board members to their boards rather than seeking out new directors.

“The increase in boardroom diversity over the last two years is encouraging, but we must not overlook that Caucasian/White men still hold 66% of all Fortune 500 board seats and 91.1% of chairmanships on these boards,” said Linda Akutagawa, chair for the Alliance for Board Diversity and president and CEO, LEAP (Leadership for Asian Pacifics).

“While progress has been achieved, there is still much more work to do,” said Deb DeHaas, vice chairman and national managing partner, Deloitte Center for Board Effectiveness.

‘Wokeness’ in the Boardroom 

Corporate America has been responsive to the wave of activism, particularly across social media, in regards to racism, sexism, economic inequality, and various other societal ills. Last year, Nike interjected itself into the heated debate over NFL players kneeling during the national anthem pre-game, to protest police brutality. The athletic apparel company made the symbol of the kneeling movement, Colin Kaepernick, a star in one of its ad campaigns.

The risk of offending customers who disagree with protesting on the field seemed to be worth taking. Nike’s sales increased 31% after the Kaepernick ad backlash.

Recently, Gillette, a Procter & Gamble brand, released an ad in line with the #MeToo movement, urging men to take responsibility for sexist behavior of other men. The ad is inciting both praise and outrage.

It’s not yet known how the controversial ad will affect P&G’s bottom line; the company is set to release its Q2 earnings next week (but so far, Wall Street speculation is favorable).

Burger King is the latest company to wade into political waters after posting a tweet poking fun at a misspelled tweet of Donald Trump’s.


CNN coined this ad trend “woke advertising.” This “wokeness” has presumably made it into the corporate boardrooms as the growing diversity board diversity numbers seem to evidence.

Despite Spate of Black Executive Board Appointments, Challenges Persist

A number of high-profile black executives have been appointed to the boards of some of the world’s largest companies. Last November, Nike announced the appointment of John W. Rogers, the CEO and founder of Ariel Investments L.L.C. to its board. Retired AMEX CEO Ken Chenault sits on the boards of Facebook and Airbnb. Edith Cooper, the executive vice president and global head of human capital at Goldman Sachs was added to Silicon Valley company Slack’s board. As progress is made, challenges remain. One issue is that most board appointments come from the C-suite level and from the pool of corporate CEOs, in particular. The number of black CEOS at the corporate level has shrunk in recent years. Chenault actually discussed this issue with BE in a recent interview.

“We have a long way to go,” said Chenault. “As I’ve said publicly, I think it’s embarrassing that the number of African American CEOs has actually been reduced from eight years ago. That’s a serious problem. From an African American perspective, we are underrepresented. We can talk all the theories we want. People talk about the complexity of this issue. I know that there are very qualified people. They just haven’t gotten the opportunity.”

While it’s important to celebrate the achievement made in diversifying American corporate boards, there is still the need to build up the pipeline of qualified black executives that can ascend to the C-suite.

Fort Bend ISD Announces Withdrawal Of Rezoning Plans For High Schools

A large concern for parents of students at Fort Bend Independent School District high schools was the potential of more rezoning and plans that could affect students in the southeast region of the district. The district announced on Friday that it will withdraw its recommendation to make any changes to high school rezoning boundaries. On Tuesday, the district’s board of trustees will be presented with a plan to keep the current attendance boundaries for those school in place for at least the next two years.

The rezoning became an issue as FBISD wanted to prioritize a new high school that was approved in the 2018 bond. To build the new high school and adjoining elementary school, voters approved the $ 1 billion project in November.

FBISD superintendent sent the following message to parents:

“Our decision to withdraw our recommendation for high school attendance boundary changes at this time is based on several factors, including the identification of a site for building High School 12 and the Board’s desire to take immediate action to secure the site. This provides a more clear path forward and will enable the drawing of boundaries for the new high school and minimize the disruption to students of rezoning to balance enrollment and rezoning again to open High School 12. Our decision was also informed by the Board’s discussion at Monday’s Board Workshop, where they expressed concerns about how frequent rezoning affects our students and communities and the need to provide more long-term solutions for high school programming and building utilization.

“During Tuesday’s Board meeting, we will ask the Board for authorization to negotiate the purchase of an 80 acre-tract of land, and will share more about how we are aggressively pursuing options that could lead to an earlier opening of HS 12, in the 2022-23 school year, instead of the 2024-25 school year.

“While we will not recommend any action to change high school attendance boundaries at this time, the board will have the authority to discuss and possibly take action on changes, should they see fit.

“More information about these plans will be shared with the Board on Tuesday night, and, at the same time, we will also share a thorough overview of our plans to expand programming at Marshall High School and Willowridge High School, as set forth in the 2018 Facilities Master Plan. We have heard concerns regarding the long-term investments in these schools to assure improved student achievement and increased building utilization, and I can see where we have fallen short in effectively engaging these communities in the planning efforts thus far. While we are asking for the approval of Early College and P-TECH programs at WHS, MHS and HHS, we will continue to work with our school communities to determine what other programming options could benefit our students.

“In closing, I want to acknowledge how appreciative I am that the community has joined with us in this journey. I am very aware that considering boundary changes is not easy, but we have committed to doing so with our community in a way that honors the engagement process and is responsive to the expectations set forth by our community and the leadership of our Board of Trustees I wish you a safe and relaxing weekend.”

Like lapel width, the rise on a pair of pants is one of those things that go in and out, or rather, up and down, over the decades. But what exactly is it?

When you buy a pair of pants, do you look for the rise? If you are new to the finer points of men’s garments, the rise may not be your first consideration. So, in this article, let’s hike up our pants and address what trouser rise means and what sort of rise is best for you.

What is Trouser Rise?

Trouser rise is measured as the distance from the front waistband between the legs to the back waistband.  This may be generically referred to as the “crotch.” Low rise pants have a smaller measurement and sit lower on the torso while higher rise pants have a higher measurement and sit higher on the torso.

This may be in the range from 7-13 inches depending on the size of the pants (smaller sizes have lower rise height).  The waistband of low-rise pants rests at the hip bones, mid-rise between the navel and hip, and high rise at the natural waistline (close to the belly button or directly on it).

To remember this easily, keep in mind the two reference points of hips (low rise) vs. belly button (high rise). Rise height matters because the amount of fabric in this area dictates how long your legs and your torso are perceived to be. A higher rise makes your legs look longer, while a low rise elongates your upper body. As we’ll discuss later, this will factor in what sort of pants rise you choose.

Pants Rise and Fashion

As mentioned in our introduction, pants rise is heavily impacted by fashion. High-rise also referred to as “long-rise” or “high-waisted,” pants have had a bad reputation for several decades because they are associated with either out-of-touch granddads or young nerds with their pants hiked up to their chests.

High-rise trousers are colloquially referred to as “old man pants” and were worn by the likes of Steve Urkel on Family Matters and Martin Short’s character Ed Grimley on Saturday Night Live. Of course, these are either intended for comedic effect or illustrate simply that the majority of people don’t think about how to dress well.

Some older men may recall that “back in the day,” high-rise trousers were the standard, and this is true. If you look at illustrations in Apparel Arts or vintage advertisements, you’ll see high-rise trousers, also with wider legs, so quite the opposite of the skinny and low fits promoted in today’s fashions.

The changeover to a lower rise is only a recent development. Ironically, along with a move to casualization, lower-rise pants are at the opposite extreme on the continuum, sometimes lying below the hip bones, or in the case of streetwear down toward the buttocks.

As with most things in life, it’s best to avoid the extremes. If you choose a mid-height rise, you can be assured that what you wear from the waist down will have enduring value. However, the middle of the pack can be boring too, so there are other factors to consider.

What Rise Should You Choose?

Consider Your Body Type

The first consideration in choosing trouser rise is your body proportions. Because there’s more fabric in the rise of high-waisted pants, they create the impression that your lower body, and especially your legs, are longer. On the other hand, low-rise pants make your upper body appear longer because your pants only start at your hips.

So, if you have a long upper body, high-rise pants will counterbalance that and make you look more proportional. On the other hand, if you are short waisted, a lower rise should be your choice to make your upper body appear longer.  Of course, if you wear a suit jacket or sport coat and keep it buttoned when you walk around, these differences will matter less.

High-Rise Pants Elongate the Legs

Similar to pleats, a high rise can make your more comfortable if you carry weight in your abdomen. Often high-rise pants will also be pleated. As a side benefit, a higher waistline also helps disguise your belly.

If you wear low-rise pants, even some holiday overindulgence at the dinner table will give you an overhang or muffin top above your waistband. This makes the tucked-in bottom of your shirt look sloppy in turn, and if you wear a knitted sweater or vest, this in combination with lower-rise trousers will visually emphasize your gut.

Higher-rise pants are therefore more flattering under these several conditions. When sizing, be aware that if your waist measurement is a certain size but you have a belly, you may need to have high-rise pants let out at the waist because it sits up where your stomach is and not at your hips.

The shape of your body also tends to change with your number of years, and pants rise is also a part of dressing appropriately to your age. While younger men have a wider range of possibilities when considering rise, an older man wearing low-rise pants risks looking like he is trying to recapture his lost youth. However, for younger men too, low rise will maintain a perception of youthfulness, but a strong motive for wearing tailoring and classic menswear is to avoid dressing like a boy.

Usually, this means not wearing flip-flops, a t-shirt and a backward baseball cap as your day-to-day outfit, but to be perfectly frank, a moderately high waist gives you a more mature appearance than pants that hang off your hips.

Check How the Pants Fit

Your decision then goes to which is more comfortable for you. When you try on a pair of pants, if the waistband is level and you feel like the crotch is bunching up or confining you, you may need a higher rise. On the other hand, if it looks like there’s excess fabric in the rise and the material is hanging down, you need a lower rise. On the internet, you can find some old tailor’s formulas for calculating the optimal rise for you based on taking your usual pants waist size, dividing by 52 and then multiplying by 18. So, if you have a 34 waist, you would calculate 34/52 x 18, giving you a rise of 11.8.” This seems about right, but try the formula for yourself.

Consider the Aesthetics of Your Look

Other aesthetic considerations come into play when choosing rise height. Note as well that high rise trousers usually will help keep your shirt tucked in as a whole simply because the shirt has a long way to rise out above the waistband. With lower-rise pants, simply moving around, bending and getting up will force you to re-tuck your shirt throughout the day.

Keep in mind that because your waistband is higher your tie will either have to be quite short to keep it just touching your waistband. Though the image of Oliver Hardy has some comic intent, such short ties were more common with the high-rise trousers of the time. Otherwise, you’d need to tuck your tie into your waistband or wear it sprezzatura style with the blades hanging below the waistband. The trick to pulling this off is to make it obvious that it is intentional.

If you like looking fashionable and contemporary, a low rise–in a slim fit, along with a fitted suit jacket or blazer–will be your choice. However, if you prefer a traditional, even vintage, look, opt for a higher rise. That’s the way men during the Golden Age of menswear wore them, and they knew what they were doing in creating a clean, uninterrupted transition from the jacket to the pants.

There should never be a small triangle of shirt showing below the buttoning point of your jacket when you have it closed as this disrupts the flow of your look. If your trouser rise is too low, that bit of shirt will certainly be visible where the quarters (front skirt) of your jacket begin to separate. You can disguise this gap with your tie, which is a little better though technically still incorrect form, but the best solution is a higher waistline; some mid-rise pants will be high enough.

Gents from the first half of the 20th century also knew high-waisted pants look best if you are wearing a suit jacket, blazer or sport coat, especially closed. If you open your jacket, high-waisted pants risk making your chest look concave or sunken and also shortens your torso. Even the Swedish sartorial icon Andreas Weinås looks better with his jacket closed when wearing high-rise trousers.

Where to Buy Mid- and High-Rise Trousers

So, ultimately, in most cases, it’s better to go with a mid- to high rise. That’s why many gents who try a higher rise say they’ll never go back to something lower. We certainly would welcome a return of popularity for the more traditional high rise on dress pants!

High Rise Pants are Rarely Available Off the Rack

In fact, it’s virtually impossible. The trend for lower rise pants, despite the fact that they are flattering for so few, means you’re less likely to find high-rise pants because they won’t appeal to a broad clientele. One possibility is to try small menswear boutiques and haberdashers. Berg & Berg, for example, only sell higher rise trousers, but the selection is fairly small.

Mid-Rise Pants, However, Are Much Easier to Find (Just Avoid Fast Fashion)

On the other hand, it’s much easier to find mid-rise pants from brands that lean classic, such as Ralph Lauren. They list the height of the rise in the “details” section of their product listings. Many of their OTR models are mid-rise, even though they aren’t advertised as such. In general, pleated pants require a higher rise to drape properly, so that feature is an indication the rise might be higher than average.

Avoid fast fashion brands entirely; they are simply too trend focused to offer mid- or high-rise pants.

A better option, though often more expensive, is to go bespoke or made to measure. Luxire is a name that often comes up when discussing custom trousers. They’ll copy a pair of pants with a rise height you like in any fabric they stock, and they have a lot. The quality is not the highest, but the fits are good.

Buy Pants Large and Tailor Them to Fit

Ethan Wong of Street x Sprezza presents interesting advice for how to get around this shortcoming. He recommends buying pants 1-2 sizes larger than you normally wear and getting the legs tapered and the waist taken in. Bigger sizes will have longer rises by default, and you can will have a larger selection to choose from if you think outside the box this way.

Conclusion

Although many men don’t think much about the rise when buying a pair of pants, those who are true sartorialists understand that it has a tremendous impact on how the trousers fit, how they look and how they feel. Though the current trend is toward lower rises, a medium to high rise usually has more benefits in terms of appearance and comfort. The choice ultimately depends on your body shape and personal taste. So, what sort of rise do you prefer?

Medicare Advantage Riding High As New Insurers Flock To Sell To Seniors

Health care experts widely expected the Affordable Care Act to hobble Medicare Advantage, the government-funded private health plans that millions of seniors have chosen as an alternative to original Medicare.

To pay for expanding coverage to the uninsured, the 2010 law cut billions of dollars in federal payments to the plans. Government budget analysts predicted that would lead to a sharp drop in enrollment as insurers reduced benefits, exited states or left the business altogether.

But the dire projections proved wrong.

Since 2010, enrollment in Medicare Advantage has doubled to more than 20 million enrollees, growing from a quarter of Medicare beneficiaries to more than a third.

“The Affordable Care Act did not kill Medicare Advantage, and the program looks poised to continue to grow quite rapidly,” said Bill Frack, managing director with L.E.K. Consulting, which advises health companies.

And as beneficiaries get set to shop for plans during open enrollment — which runs from Monday through Dec. 7 — they will find a greater choice of insurers.

Fourteen new companies have begun selling Medicare Advantage plans for 2019, several more than a typical year, according to a report out Monday from the Kaiser Family Foundation. (KHN is an editorially independent part of the foundation.)

Overall, Medicare beneficiaries can choose from about 3,700 plans for 2019, or 600 more than this year, according to the federal government’s Centers for Medicare & Medicaid Services.

CMS expects Medicare Advantage enrollment to jump to nearly 23 million people in 2019, a 12 percent increase. Enrollees shopping for new plans this fall will likely find lower or no premiums and improved benefits, CMS officials say.

With about 10,000 baby boomers aging into Medicare range each day, the general view of the insurance industry, said Robert Berenson, a Medicare expert with the nonpartisan Urban Institute, “is that their future is Medicare and it’s crazy not to pursue Medicare enrollees more actively.”

Bright Health, Clover Health and Devoted Health, all for-profit companies, began offering Medicare Advantage plans for 2018 or will do so for 2019.

Mutual of Omaha, a company owned by its policyholders, is also moving into Medicare Advantage for the first time in two decades, providing plans in San Antonio and Cincinnati.

Some nonprofit hospitals are offering Medicare plans for the first time too, such as the BayCare Health system in the Tampa, Fla., area.

While Medicare beneficiaries in most counties have a choice of several plans, enrollment for years had been consolidated into several for-profit companies, primarily UnitedHealthcare, Humana and Aetna, which have accumulated just under half the national enrollment.

These insurance giants are also expanding into new markets for next year. Humana in 2019 will offer its Medicare HMO in 97 new counties in 14 states. UnitedHealthcare is moving into 130 new counties in 13 states, including for the first time Minnesota, its headquarters for the past four decades.

Extra Benefits

Seniors have long been attracted to Advantage plans because they often include benefits not available with government-run Medicare, such as vision and dental coverage. Many private plans save seniors money because their premiums, deductibles and other patient cost sharing are lower than what beneficiaries pay with original Medicare. But there is a trade-off: The private plans usually require seniors to use a restricted network of doctors and hospitals.

The federal government pays the plans to provide coverage for beneficiaries. When drafting the ACA, Democratic lawmakers targeted the Medicare Advantage plans because studies had shown that enrollees in the private plans cost the government 14 percent more than people in the original program.

Medicare plans weathered the billions in funding cuts in part by qualifying for new federal bonus payments available to those that score a “4” or better on a five-notch scale of quality and customer satisfaction.

Health plans also gained extra revenue by identifying illnesses and health risks of members that would entitle the companies to federal “risk-adjustment” payments. That has provided hundreds of billions in extra dollars to Medicare plans, though congressional analysts and federal investigators have raised concerns about insurers exaggerating how sick their members are.

A study last year found that those risk adjustments could add more than $ 200 billion to the cost of Medicare Advantage plans in the next decade, despite no change in enrollees’ health.

For-profit Medicare Advantage insurers made a 5 percent profit margin in 2016 — twice the average of Medicare plans overall, according to the Medicare Payment Advisory Commission, which reports to Congress. That’s slightly better than the health insurance industry’s overall 4 percent margin reported by Standard & Poor’s.

Those profit margins could expand. The Trump administration boosted payments to Medicare Advantage plans by 3.4 percent for 2019, 0.45 percentage points higher than the 2018 increase.

Betsy Seals, chief consulting officer for Gorman Health Group, a Washington company that advises Medicare Advantage plans, said many health plans hesitated to enter that market or expand after President Donald Trump was elected because they weren’t sure the new administration would support the program. But such concerns were erased with the announcement on 2019 reimbursement rates.

“The administration’s support of the Medicare Advantage program is clear,” Seals said. “We have seen the downstream impact of this support with new entrants to the market — a trend we expect to see continue.”

Getting Consumers To Switch

Since the 1960s, Mutual of Omaha has sold Medicare Supplement policies — coverage to help beneficiaries in government-run Medicare pay the portion of costs that program doesn’t pick up. But the company only briefly entered the Medicare Advantage business once — in its home state of Nebraska in the 1990s.

“In the past 10 or 20 years it never seemed quite the right time,” said Amber Rinehart, a senior vice president for the insurer. “The main hindrance was around the political environment and funding for Medicare Advantage.”

Yet after watching Medicare Advantage enrollment soar and government funding increase, the insurer has decided now is the time to act. “We have seen a lot more stability of funding and the political tailwinds are there,” she said.

One challenge for the new insurers will be attracting members from existing companies since beneficiaries tend to stick with the same insurer for many years.

Vivek Garipalli, CEO of Clover Health, said his San Francisco-based company hopes to gain members by offering low-cost plans with a large choice of hospitals and doctors and allowing members to see specialists in its network without prior approval from their primary care doctor. The company is also focused on appealing to blacks and Hispanics who have been less likely to join Medicare Advantage.

“We see a lot of opportunity in markets where there are underserved populations,” Garipalli said.

Clover has received funding from Alphabet Inc., the parent company of Google. Clover sold Medicare plans in New Jersey last year and is expanding for 2019 into El Paso, Texas; Nashville, Tenn.; and Savannah, Ga.

Newton, Mass.-based Devoted Health is moving into Medicare Advantage with plans in South Florida and Central Florida. Minneapolis-based BrightHealth is expanding into several new markets including Phoenix, Nashville, Cincinnati and New York City.

BayCare, based in Clearwater, Fla., is offering a Medicare plan for the first time in 2019.

“We think there is enough market share to be had and we are not afraid to compete,” said Jim Beermann, vice president of insurance strategy for BayCare.

Hospitals are attracted to the Medicare business because it gives them access to more of premium dollars directed to health costs, said Frack of L.E.K. Consulting. “You control more of your destiny,” he added.

Breast Cancer Awareness Month in October presents another opportunity to spread information about the disease and how it affects Black women. The disease is responsible for a high death rate in women of color, according to the Centers for Disease Control and Prevention.

Black women are 40 percent more likely to die from breast cancer despite doctors diagnosing the disease in African-American and white women at about the same rate, the CDC reported. Also, Black women are more often found to have triple-negative breast cancer, an aggressive type that frequently returns after treatment.

Age is also a big factor: breast cancer incidence rates were higher among African-American women younger than 60 years old but lower among those who are 60 or older.

One reason for this statistic could be that medical professionals tend to find this cancer at an earlier stage in white women.  Also, Black women may have inadequate medical care, including limited access to cancer screening technology.

Doctors encourage women, especially those at high risk due to a family history or having BRCA1 or BRCA2 genes, to get out ahead of the disease. Early detection measures such as mammograms and screenings are strongly recommended. Women can visit local hospitals that offer low-cost mammograms or call their local American Cancer Society chapter for help with screenings or doctor referrals.

In addition, researchers continue to look into why some women are more susceptible to triple-negative breast cancer, in order to find better treatment options.

Women can also choose a healthier lifestyle for a better chance of lowering risks for the disease. BreastCancer.org recommends exercise, a nutritious diet and avoiding smoking and alcohol as important in the battle against breast cancer — one that many women can win regardless of race.
